Jason Rice, founder and CEO of My Med List, is a licensed pharmacist in Florida and North Carolina, and possesses over 26 years of experience as a pharmacist and over 33 years as a healthcare professional. Over the past 2 years, he has served as a hospital director of pharmacy with a passion for patient and medication safety. Additionally, he served 13 years as System Director of Pharmacy for a major healthcare system in northwest North Carolina where he designed and implemented the its Medication Reconciliation Program. This experience allowed for the identification of major gaps in the transition of care for patients, which unfortunately have led to tremendous numbers of medication errors and unintended patient harm. My Med List aims to change this broken part of the healthcare system by giving patients the attention they deserve and the tools they need to allow for greater peace of mind when it comes to managing their medications and overall health.
